目的:探讨建立一种放射治疗全身器官剂量数据库平台的可行性。方法:使用基于深度学习的自动勾画软件DeepViewer?1例食管癌患者的全身CT上勾画全身器官,然后利用基于GPU加速的蒙特卡罗软件ARCHER计算相应的器官剂量分布,最后利用Lyman-Kutcher-Burman(LKB)模型评估放疗患者正常组织并发症概率(NTCP)。结果:针对该病例,成功建立基于DeepViewer?ARCHER和LKB模型的全身器官剂量数据库,发现距离靶区越近的器官剂量越大,其中心脏与靶区间距离最小,剂量为14.11 Gy,但因其模型参数特殊,通过LKB模型计算的NTCP为0.00%;左、右肺的剂量分别为3.19和1.16 Gy,但是NTCP值却很大,分别为2.13%和1.60%。对于距离靶区较远的头颈部器官(视交叉、视神经和眼)和腹部器官(直肠、膀胱和股骨头)剂量分别约为9和2 mGy,并且NTCP均近似为0.00%。结论:研究结果证明通过自动勾画软件DeepViewer?蒙特卡罗软件ARCHER和LKB模型建立全身器官剂量数据库的可行性。  相似文献

目的比较胸腰椎骨折患者接受经椎旁肌间隙入路与微创经皮入路手术治疗的效果及关节功能。方法回顾性分析2014年4月-2015年10月在该院接受内固定手术治疗的胸腰椎骨折患者,根据手术入路不同分为椎旁入路组和经皮微创组,分别接受经椎旁肌间隙入路与微创经皮入路手术治疗。比较两组患者的手术情况、围手术期血清创伤分子含量以及远期功能恢复情况和矫正畸形效果。结果椎旁入路组的手术时间明显短于经皮微创组,差异有统计学意义(P0.05);术中出血量、术后引流量、术后卧床时间、住院总时间与经皮微创组比较,差异均无统计学意义(P0.05);两组患者术后当天的血清肌酸激酶(CK)、肌红蛋白(Myo)和乳酸脱氢酶(LDH)含量比较,差异均无统计学意义(P0.05),术后3、6和12个月时的伤椎前后缘高度比、Cobb角、视觉模拟评分(VAS)比较,差异均无统计学意义(P0.05)。结论椎旁肌间隙入路与微创经皮入路手术治疗胸腰椎骨折的效果和创伤程度相当,椎旁肌间隙入路的手术操作更为简单、手术时间更短。  相似文献

Chronic hemolysis and intermittent vaso-occlusion in sickle cell disease can lead to recurrent blood transfusions with related complications such as antibody formation and rarely, life-threatening reactions. We report a case of a parturient who presented with complications of sickle cell disease and who later had fetal compromise that required an emergent cesarean delivery. Complex management decisions were made, aided by technologies such as rotational thromboelastometry, quantitative blood loss analysis and cell salvage.  相似文献

目的探析对剖宫产产妇采取中西医结合护理的效果。方法本次研究72例剖宫产产妇均为本院2018年3月-2018年11月接收,随机分为2组,对照组(n=36)行常规护理,研究组(n=36)开展中西医结合护理,统计各组护理效果。结果在初乳泌乳始动优良率、母乳喂养、乳汁分泌充足及乳房胀痛发生率方面,研究组均优于对照组,差异有统计学意义(P<0.05)。结论中西医结合护理在剖宫产产妇护理中效果突出,有助于改善产妇术后的乳汁分泌情况,值得推广。  相似文献

Objective: For Arabian traditional medicine, Crataegus aronia syn. Azarolus (L) Bosc. ex DC (Rosaceae) is widely used to treat diabetes, sexual weakness, cardiovascular diseases and cancer. The anti-cancerous and anti-hemolysis effects of the hydroalcoholic extract of this plant have never been investigated before. The present study aims to evaluate the biological activities of the hydroalcoholic extract of Crataegus aronia leaves in combination with cisplatin, one of the most widely employed chemotherapeutics, on A549 human lung cancer cell line. Methods: The anti-oxidant and anti-proliferative activities of leaves, fruits, seeds of C. aronia were investigated by DPPH method and MTT assay; respectively. Cell migration activity was investigated by wound healing and by cell aggregation assays. The effect of C. aronia in inducing cell cycle arrest along with activating cell apoptosis was evaluated by flow cytometry and Western blot assays, respectively. Results: Our results showed that C. aronia leaves (C. aronia L.) had the highest anti-oxidant and anti-proliferative activities. The leaves extract was potent against hemolysis of the human erythrocytes and showed elevated decrease in migration by reducing wound healing migration and by increasing cell aggregation. Finally, C. aronia L. treatment exhibited apoptotic activity on A549 cells by the down-regulation of PARP-1, caspase-3 and Bcl-2 proteins and by increasing the percentage of A549 cells in sub G0 cell cycle. Moreover, the co-treatment of C. aronia L. and cisplatin remarkably sensitised A549 cells to cisplatin. Conclusion: The results suggested that C. aronia L. could be used as a potential treatment against human lung cancer exhibiting minimal side effects on human health.  相似文献

BackgroundNorepinephrine is as effective as phenylephrine for management of spinal anaesthesia-induced hypotension. Most of the studies comparing these vasopressors have been conducted in healthy pregnant women undergoing elective caesarean section. In the current study, we tested the null hypothesis that there is no difference in neonatal outcome when phenylephrine or norepinephrine is used to treat spinal anaesthesia-induced hypotension in women undergoing emergency caesarean section for fetal compromise.MethodsPatients undergoing caesarean section for fetal compromise who developed spinal anaesthesia-induced hypotension were randomised to receive phenylephrine 100 μg or norepinephrine 8 μg for treatment of each hypotensive episode, defined as systolic blood pressure <100 mmHg. Umbilical cord arterial and venous blood samples were obtained for blood gas analysis. The primary outcome measure was umbilical artery pH.ResultsOne hundred patients (50 in each group) were studied. There was no significant difference in umbilical artery pH between the two groups (mean difference 0.001; 95% CI −0.032 to 0.034). The number of hypotensive episodes, vasopressor boluses required, the incidence of bradycardia, heart rate and blood pressure trends following vasopressor administration, and the incidence of nausea/vomiting were not significantly different between groups.ConclusionPhenylephrine 100 μg and norepinephrine 8 μg were not significantly different in terms of neonatal outcome when administered as intravenous boluses for treatment of spinal anaesthesia-induced hypotension in parturients undergoing emergency caesarean sections for fetal compromise.  相似文献
